If you want to enter more than three athletes or events, you may submit a spreadsheet with your information instead of creating an entry for each individual.

Step 1: Click here to download the official team registration form.
PLEASE NOTE THIS IS A NEW FORM FOR 2025. DO NO REUSE FORMS FROM LAST YEAR.
Complete the form and save it on your device. YOU MUST SUBMIT YOUR ATHLETES' INFORMATION IN THE 2025 OFFICIAL FORMAT.
NOTE: If you are NOT paying by credit card, stop here. Email your completed team registration form to hammermanusainfo@gmail.com If you are paying by credit card, continue with step 2.
Step 2: Enter total number of registrations for EACH event in the boxes below.
Step 3: Click the Add to Cart button. You will be taken to your cart page. If everything looks right, click CONTINUE TO CHECKOUT.
Step 4: Click the button below to email your spreadsheet to hammermanusainfo@gmail.com.
YOUR ATHLETES' SPOTS IN THE COMPETITION ARE NOT RESERVED UNTIL WE RECEIVE YOUR SPREADSHEET IN THE REQUIRED OFFICIAL 2025 FORMAT.
